English term or phrase: Informed assent form
Can someone help me with this please: in clinical trials in English there is a difference between an "informed consent form" (for people over the age of consent) and an “informed assent form” (for minors). Is a similar distinction drawn in Slovak or do you just have “súhlas” for both consent and assent?

Explanation:
There is no such distinction drawn in Slovak. The State Institute for Drug Control in Bratislava uses only "informovaný súhlas / formulár informovaného súhlasu".

Simply adding "for minors" in brackets, if applicable and/or necessary, could be used instead.

Explanation:
I believe that a difference between the informed consent/assent forms for adult/minor participants in clinical trials exists even in the Slovak language.

The term in question can be found used on page 6 of the reference text (source: The State Institute for Drug Control in Bratislava).

We do not distinguish between "consent" and "assent". If it is intended for minors, just add "pre neplnoletých účastníkov skúšania".
