I am working on translating some marriage documents from the Philippines (originally in English) to Japanese and am having issues with this form. The original marriage document I have found an official Japanese translation, so I that has made most of it really easy, but this Affidavit part I am having trouble.

We're not sure who (a Catholic priest, city mayor, judge, etc.) is marrying who, so it would be safe to say 執行役員. If a priest/reverend/pastor/imam/rabbi is marrying the couple, we could say 司式者 instead of 執行役員.
