So, what could have happened in the process is fracturing of the fibula with the distal fibula fragment pointing laterally. De weberclassificatie bestaat uit type A, B en C. – Een A-fractuur bevindt zich distaal van de syndesmose. Aangezien de syndesmose hierbij intact blijft en de enkelvork dus congruent, is chirurgisch ingrijpen zelden geïndiceerd. Bij onvoldoende consolidatie (non-union) van de fractuur kan later het distale fragment –afhankelijk van de grootte – worden verwijderd of gefixeerd. – Bij een B-fractuur bevindt zich de fractuurlijn op syndesmoseniveau. Deze fractuur kan stabiel of instabiel zijn. Stabiele fracturen kunnen zowel conservatief door middel van gipsimmobilisatie als operatief behandeld worden. Bij een instabielel B-fractuur is er sprake van een laesie in de posterieure syndesmose, een fractuur van de posterieure (malleolus tertius) of mediale malleolus. Röntgenologisch kan er dan verplaatsing (verkorting en exorotatie) van het distale fibulafragment optreden waardoor de enkelvork incongruent wordt, met een verbreding van de ruimte tussen mediale talus en mediale malleolus tot gevolg. Dit leidt tot persisterende pijnklachten en een grotere kans op artrose. Bij een instabiele fractuur is operatieve repositie en interne fixatie geïndiceerd. – C-fracturen bevinden zich proximaal van het niveau van de syndesmose.
