Сразу скажу, что я не сторонник иностранных терминов, если есть русские аналоги. Но в данной ситуации меня заинтересовано происхождение термина vigilance, и выяснилось, что корни латинские, что "повышает" шансы его применения в медицине.

Если говорить о том, как отличить значение терминов, которые действительно часто переводятся как синонимы, то мне кажется наиболее подходящей такая схема: внимательность (способность замечать новое, улавливать изменения, пр.) и вигильность (способность сосредоточить внимание). Допустим, человек может быть внимательным (наблюдательным), но способность сосредоточить внимание может быть снижена.
