Source text - English Reconstruction of gastrointestinal continuity may be accomplished by reconnecting the stomach directly to the duodenum or by closing the duodenal stump and creating a gastrojejunostomy. A Billroth I anastomosis requires at least 1 cm of healthy duodenal wall above the pancreas and cannot always be accomplished. However, the Billroth I reconstruction avoids the complications associated with the duodenal stump, afferent and efferent limb syndromes, and formation of marginal ulcers. In general, reconstruction with the gastroduodenostomy (Billroth I) is advocated over a gastrojejunostomy (Billroth II) because it seems to be more “physiologic,” minimizing dumping and weight loss.
A Billroth II reconstruction is favored if inflammation of the duodenum makes construction of a Billroth I technically difficult. For the Billroth II, a loop of jejunum is anastomosed to the gastric remnant, as described earlier for gastrojejunostomy. The duodenum is oversewn or stapled carefully, because leaks result in a high mortality. Nasogastric tube decompression through the gastrojejunostomy postoperatively may be protective.
