When deciding between easy and pro, err on the side of pro. Most questions will be pro.

Último posible contacto... Me parece que "conocido" es innecesario. Con identificarlo como "último" se sobreentiende el resto.

I suggest "último contacto probable conocido"

I think that "probable" fits better than "posible" in this context
