This term does not have a perfect translation, as the specifics which it implies are unique to the Spanish educational system. Given that Matricula and Honor are cognates for matriculation and honor, I´d leave it in Spanish and explain it in an appositive phrase or in parentheses. The other options suggested, Im afraid, are inaccurate. It´s not just with distinction, or honor roll, or Dean´s list, etc. It is higher than any of those as a distinction. Another option is to footnote the term and explain it in a footnote: "In Spain the Matricula de Honor is the highest grade issued in a course and in many cases grants the student earning it a waiver of his tuition fees."
