Others here are on the right track, but I'd try and make sense of the whole description of the various pathological symptoms:
"Some [of these] acute episodes were characterised by strong feelings of alienation, uncontrolled movements and hallucinations. Others were brief periods of extreme mental confusion accompanied by terrible nightmares."
Something like that.
