When we use corneal topography to look for ectatic disease, the goal is not to fit the shape to corneal abnor- malities, but to accentuate them. Renato Ambrósio Jr, MD, PhD, and I worked for a long time to choose an optimal shape for the topographic reference point that would be representative of the cornea as well as intuitive for the clinician. Although any reference surface can be used, the best surface is one that easily and understand- ably conveys clinically relevant information, such as astig- matism and areas of ectasia. Based on these criteria, we chose a best-fit sphere for the topographic reference
point. We currently use an 8-mm optical zone to com- pute the best-fit sphere, because this size blends the cornea's peripheral flattening and the central steepening to create an easy-to-visualize map in which normal corneas look normal and abnormal corneas stand out (Figure 1). The other justification for an 8-mm optical zone is that we usually limit our displays to 9 mm. We tell surgeons not to use maps with extrapolated data (either black dots or white areas, depending on the user setting). If we have a full map on the 9-mm display, then we know that the best-fit sphere data based on the central 8 mm will be based on valid data.

Cuando se utiliza la topografía corneal para buscar una enfermedad ectásica, la finalidad no es encajar la forma de las irregularidades corneales, sino acentuarlas. El Dr. Renato Ambrósio Jr, PhD, y el Dr. Michael Belin trabajaron por varios años eligiendo la forma más óptima para el punto de referencia topográfico que sería representativo de la córnea como también intuitivo para el médico clínico. Aunque pueda usarse cualquier superficie de referencia, la mejor superficie es aquella que expresa, de forma fácil y entendible, información clinicamente relavante, como el astigmatismo y las áreas de ectasia. Al basarse en estos criterios, se eligió una esfera que mejor se adapte el punto de referencia
topográfico. Actualmente se utiliza una zona óptica de 8 mm para calcular la esfera que mejor se adapta, ya que este tamaño permite mezclar el aplanamiento periférico de la córnea y la curvatura central para así crear un mapa de fácil visualización donde las córneas normales se vean normales y las anormales resalten. (Figura 1). Otra razón para utilizar una zona óptica es debido a que, generalmente, las imágenes se limitan a 9 mm. Se les indica a los cirujanos no utilizar mapas con datos extrapolados (ya sean puntos negros o áreas blancas, dependiendo de las configuraciones del usuario). Si se tiene un mapa completo en una imagen de 9 mm, entonces se concluye que la información de la esfera de mejor ajuste basada en los 8 mm centrales estará basada en información válida.
