I'm filling out a Swedish passport application for my new son (I live in the US) and my Swedish is quite rusty. These are the phrases on the BACK of the passport application so I'm wondering if they're applicable.

Förlustanmälan means reporting loss of passport.
Medgivande underårig/??? underårig=under aged
Försäkran om svenskt medborgarskap=proof of Swedish citizenship

I'm in the process of applying for passports for my kids as well, except I'm doing this from Canada and my kids are 9 and 20. I can't imagine the difference being too big. In short, what's required here:
- Birth certificate (US in your son's case)
- Record of citizenship (something the Swedish consulate will take care of)
- In my case, proof that I'm not a Canadian citizen.Written concent from both parents.

I'm sure the Swedish Consulate would be able to clarify things for you too.

Sounds like the back of the form is asking why you are applying for the passport (did you lose it, are you underage, etc.) It's hard to tell without more context
