Faultから安全要求の侵害に至るまで

This is the title of a section from a document I'm translating.

I'm not sure how loose or literal of an approach I should take for this particular phrase.

My general sense is something like: "How faults lead to safety requirement infringements"

How does this sound? Or can someone offer a 'cleaner' or more natural sounding translation?

What you proposed is one interpretation of the phrase. Look through the section. Does it list faults or processes which may lead to OHS violations? If so, your translation is correct and depending on the conventions you adapted for the document you should stick to "How faults lead to safety requirement infringements" or perhaps simply "Faults leading to OHS violations".



Another option is the common usage of -から -に至るまで as "ranging from x to y". In this case the phrase would mean "Failure modes ranging from faults to safety risks", the implication being that faults are less severe and OHS risks are more severe. Looking through the section, does it list a variety of failure modes, both faults and OHS risks (possibly something in between if we assume the contiguous range of severity), not necessarily as a cause of one another but instead as individual instances? If so, I would suggest sticking to this meaning instead. Consider something along the lines of "Risks ranging from faults to OHS violations".
