This is one of many consent and privacy forms for a British patient's electronic medical records. I am unsure what type of document "atto" refers to, and what "proposto" means in this context.

@ Judy That is useful to know, but this document says "atto sanitario". The patient is being treated in the diabetic footcare department, and seems to have quite a lot of ongoing treatment for foot-related issues triggered by diabetes. The handwriting in this particular form is barely readable, but I can make out the word "blocco", so perhaps this is for blood vessel issues....?

It might be useful to know what the proposed procedure was, as Italian distinguishes
between "atto sanitario" (healthcare-related procedure) and "atto medico" (can only be done by doctors). It might sound less natural, but I'd maintain the distinction, especially in a consent form
