I don´t know whether it fits your context, and there were also a lot of hits for Akut Plejeenhed.
I frequently translate epikirise as ´discharge summary´ - as long as it fits, of course! I find it frequently used when a patient is discharged or transferred from one department to another, picking up the main points and rounding off ´the story so far´.
