Italian to English Medical Medical (general)

In the past history of a medical report. Probably just shorthand for an obvious Italian phrase that just escapes me at present.

exeresi di adenoma ipofisario nel 1989 (rtp adiuvante successiva)
protesi non metallica anca dx nel 2004
etc.
