For each one under the subheading HARGA PEROLEHAN is listed an amount of money. In the final column there is a subheading KETERANGAN under which it says "tahun akhir"
The literal translation "Acquisition Price" does not make sense because the tax payer has had these savings accounts for several years. The term Harga Perolehan seems to indicate the amount in the savings account.
