I do not speak Ukrainian but I need to translate this phrase into English.

Just want to make sure that my version is ok:
Kyiv Pecherskyi District State Civil Registration Department of the Central Interregional Department of the Ministry of Justice
But why is there the city of Khmelnytskyi in brackets, if the organisation is in Kyiv?
Any help with that would be highly appreciated.
