The case concerns a bypass patient fitted with a PM who has been admitted to the day hospital for tests due to worsening heart failure etc.
I'm just not sure how to translate 'Apertura di Day-Hospital' - is it simply a way of indicating that the case was opened in the day hospital?

I think it refers to a phase of the patient's treatment, to address the current issue, that was transferred over to staff at the day hospital. The beginning of tests, or of the initial tests or testing phase for the new problem, associated with the initial condition.

I think what it means, strictly speaking, is that a file on the patient was opened on the day hospital system. It sounds to me like a line of text created by the medical file software. I see it as being similar to "presa in carico". I'm not sure that the nitty-gritty of the operating system matters for a translation of the patient's notes, but you'll be able to judge that better than us since you have the source text in front of you.
