A well-understood colloquial expression На сегодня прямого русского аналога термину long-hauler мы не имеем.
Термин этот новый, меткий, кооптирован из просторечной лексики.
Столь же просторечный термин "долгоиграющий больной" недвусмысленно указывает на природу обозначаемого феномена.
Предлагаю в качестве кандидата.

1. "Долгоиграющий", a technical term for a long-playing grammophone record, introduced into Russian in the 1950s, has long been used figuratively (see https://ru.wiktionary.org/wiki/долгоиграющий 2.)

2. The volume of material available online, since March 2020 (google searches yield dozens of hits quoting Italian, French and British sources) should strenghten translator's confidence that the result will be understood by general readers and professionals.

3. For a shorter form of "long-hauler" I'd propose something which I haven't yet seen 'in the wild' - "долгоиграющий ковидный больной" (see my discussion note).
