On 10/4/18, FSW received a request to follow up on [REDACTED – SECOND CHILD’S FIRST NAME] and [REDACTED – FIRST CHILD’S FULL NAME]'s lice problem at school with the nurse. FSW spoke to the school nurse who was very familiar with the children's lice problem. The nurse stated lice has been an ongoing issue for this family for the past two school years. In the presence of the FSW, the nurse combed through the both [REDACTED – FIRST CHILD’S FIRST NAME] and [REDACTED – SECOND CHILD’S FIRST NAME]'s hair and found lice and larve (nits) in their hair.
