This abbreviation appears in a set of hand-written nursing notes (this entry all in caps).
"PZ LAMENTA ALGIE IN SEDE D'IMPIANTO. SOM DR XXX SI SOMMINISTRA BRUFEN 600 mg."
My guesses are "su ordine medico (+ name)" "secondo ordini del medico", "sotto osservazione medica" (perhaps too limiting for a nurse).
Does anyone know exactly what SOM stands for and what the English equivalent is?
