According to IATE, it's a Vision Test, but that sounds far too general.

Point taken but, at least in US contexts, and based on my own experience with and observation of the way optometrists do business, they do not seem to differentiate between "initial" and "follow-up" eye exams. I would also recommend that, if you do want to draw this distinction, it would make sense to do so in a single mention (i.e., "initial and follow-up eye exams") rather than separating the two services with other items on the list, as in the original (which looks odd). Best. :)
