I know that i.V.= in Vertretung= p.p., but it feels strange to say "p.p. senior physician". Should I say "acting attending"??

: Yes, it stands for "in Vertretung", but it doesn't translate as "by proxy" - that would imply that he is substituting or deputising for someone, whereas in fact he is just signing on behalf of the Klinik

: I think you are right: some (senior) Assistenzarzt temporarily acts as Oberarzt (while the latter is on vacation, maternity leave, or whatever). Could "Acting senior/registrar/..." be an option?
