This agreement is about "privacy" - that world is normally used when referring to personal data. The samples of "Privacy Disclosure Agreement" I could find on the Web are all about the personal data of the individual signing this kind of agreement - the other side being anyway a company or some state institution with no "personal" data to protect.
Still, without more context, it’s only the most likely option – you can’t be 100% sure of the meaning of a title without at least a glimpse at the content. But this being a legal document and not journalism, you would expect a title reflecting what would be usual the content under such title.
