“Cuidador” es un adjetivo, no un sustantivo, aunque hay un uso poco cuidado en el que está nominalizado. Por eso parece mejor el “explicao” “proveedor de cuidados”. Aunque en este contexto, “asistente [social]” podría ser pertinente también.
“out-of-home” referido a cualquier servicio, sería “externo”.
