Wenn ich es richtig verstehe geht es geht doch in der Sache nur darum, dass eine Leistung (Katheterwechsel) von dem, der die Leistung erbringt (Arzt, Krankenhaus) über die KK abgerechnet wird, oder?

If the wider contact shows that the KK was at fault, then claim against may be more appropriate.

I agree (after working with them every day), but there is no exact equivalent term in English. It may be that "health insurance provider" is more appropriate, depending on the text's purpose. "Krankenkasse" would be ideal if it would be understood.
