Again, a membership record form for an unknown organization. The terms "Endelig udslettet/udmeldt" appear in a column regarding health insurance, medical exams, and fees paid (presumably for the health insurance). The cell immediately beside this one contains a long list of years, handwritten. The form contains dates from the 1930s through to early 1960s. I'm guessing that Endelig udmeldt is something along the lines of final resignation, or quitting the organization. But udslettet I can't figure out a reasonable translation for.
