sorry, it came out as two words, but I think it's supposed to be 'caseworker'.
The caseworker was assigned to her, and he provide her with advocacy, information or other services.

: to me a 'personal coach' is something rich people spend a lot of money on. A caseworker is something totally different. It's refering to the person who is responsible for following a person (usually in a government institution) from start to finish

Maybe this will help to make the def of caseworker more clear, and help you decide if this is exactly what you mean.
I must admit that this is one I hadn't heard before!
I forgot the ones I use most: integration path counsellor/integration counsellor (I often work in social integration and refugee environment) and personal consultant (for VDAB, Flemish Job Search and Training Agency).
Personally, I am not in favor of case worker, project manager nor project supervisor as suggested by our collegues. "Trajectbegeleiding" is all about people, the suggested translations make me feel as if the people have become "things".
