Here again, I do not understand "avancer le vider a la main".

This describes the cautious filling of the syringe by pulling the plunger with one hand and holding it with the other. The vacuum created is thus under slow careful control to avoid it increasing too rapidly; the blood thereby fills the syringe in response in a measured way.
