Lines 6 onward: payments made & signatures

What does "szül" mean in this context? That the policy holder is "engendering" a policy for his wife?

Thanks to Denny & MandC. I gave Denny the points because Née is exactly what I needed for the doc, but obviously you both gave correct answers. I hate having to make calls like that! Anyway, this should have been obvious to me, but the whole doc is oddly written, and I couldn't figure out what birth had to do with anything! Now, I see it clearly. Thanks!
