The context is a list of items / healthcare services covered by health insurace companies. After the table, this sentence is included:
situações em branco nao está especificado em contrato pelas prestados de serviços
Empty forms are not accepted by Healthcare services providers.
Is this correct? Situacoes is sort of misleading. And there is a typo, it should "estao especificadas" instead of "está especificado".

Maybe this is a clearer way to state it, since the forms are not empy, but only some spaces are blank. Compare with this example:
