Comment: "Ti ringrazio moltissimo perché credo che sia proprio linfonodo paraaortico. Infatti più avanti nel testo avevo la frase Node at pancreas margin - free from tumour.
Quindi non so se si possa considerare una metastasi, ma sicuramente direi che è un nodulo paraaortico. Altra spiegazione non c'è. Sto comunque attendendo una risposta anche dal medico, per questo ho atteso un po' a dare la mia votazione. Grazie a tutti quelli che hanno risposto"

presumo si tratti di PanIN-1, senza grade non avrebbe molto senso indicarlo

To evaluate the uniformity of existing systems for grading duct lesions in the pancreas, 35 microscopic slides with 35 representative duct lesions were sent to eight expert pathologists from the United States, Canada, and Europe. Kappa values for interobserver agreement could not be calculated initially because more than 70 different diagnostic terms were used by the eight pathologists. In several cases, the diagnoses rendered for a single duct lesion ranged from "hyperplasia," to "metaplasia," to "dysplasia," to "carcinoma in situ." This review therefore demonstrated the need for a standard nomenclature and classification system. Subsequently, during a working group meeting, the pathologists agreed to adopt a single standard system. The terminology **pancreatic intraepithelial neoplasia (or PanIN)** was selected, and diagnostic criteria for each grade of PanIN were established (http://pathology.jhu.edu/pancreas_panin). This new system was then evaluated by having the eight pathologists rereview the original 35 cases. Discussion
Io mi sono basata piuttosto sulla sorta di riassunto riportato pi� avanti nel testo dove si diceva Node at pancreas margin - free from tumour, che mi sembrava fosse la ripetizione di questo PAN-1n at pancreas margin trovato all'estemporanea. Non so se i miei ragionamenti siano completamente giustificati, anche perch� purtroppo non ero presente alla dimissione, altrimenti avrei potuto chiedere direttamente. Histology: Provisional Diagnosis: ypT4, ypN1 (17/34), Mo. Duodenal adenocarcinoma. Closing results give macroscopic evidence for a duodenal carcinoma. The differentiation pattern gives evidence for a pancreatic carcinoma. For that it is not clear to state of what origin the tumour is.
Macroscopic- 4.5 cm diameter polypoid tumour in the duodenum superior to the papilla with invasion of the papilla and pancreas.
Microscopic- Middle grade poorly differentiated adenocarcinoma of the duodenal mucosa with significant tumour necrosis and infiltration of the duodenal papilla, pancreas and peri-pancreatic fat. 16/24 regional lymph nodes involved. Perineural tumour propagation. All tissue margins free from tumour. Gallbladder-chronic cholecystitis. No evidence of tumour in the resected old jejunal roux-en-y loop or the accompanying jejunal mesenteric lymph nodes. ***Evidence of PAN-1n at pancreas margin (Schnell-schnit)
Quello che mi sembrerebbe strano � che i dottori hanno detto che non ci sono metastasi.
Schnellschnitt: il medico che ha redatto questa lettera � americano o australiano, ma l'ospedale che ha fatto l'intervento � tedesco. In effetti anche questo Schnellschnitt mi pone problemi.
