I cannot understand the use of napravlyenii here.
Context: Survey carried out among drug addicts to establish how many youth are drug addicts.
The full sentence is: The percentage of youth among people, ookazannikh respondentami v blankakh napravlyenii, when carrying ouy research among drug addicts, was at least 50%.
It seems that the drug addicts are putting down names on a form.

Comment: "It was hard to pick just one answer! 'Referral form' is most suitable for a medical context in English. But Ann's explanation was most helpful.
In any case it still wasn't clear how exactly the drug users were being 'surveyed' - in person (by outreach workers) or just using data from forms. I just highlighted the ambiguity for my client."

usual survey in russian polulinics does not mean direct asking of patients. It is conducted by looking through statistics records kept in archives= В РЕГИСТРАТУРЕ.

I agree with Sergej and it isn't really easy to explain the system in Russia,Ukraine,etc. to foreinger.So, it'll be the form to direct or send that guy wherever. Usually the person,who gave it to the addict-he/she supposed to fill the data(not the addicts,they just answered the questions):first and last name, age,sex and what was the reason for filling the form.
In this point I don't think that these forms for the treatment - it'll be so sweet, like a dream! 99% from them don't want to be treated and now the treatment against their wish(принудительное лечение)is forbidden.So, it isn't about this. More likely they need to examine something, take blood tests,etc. There is something to scare them to death- when they knew that anybody from the group became HIV-positive.They want to check their own blood and it is the most possible way to fill the form and get the data.Then the person,who does research,comes to the laboratory(usually there are not more than 3-5 even in a big cities),takes the form(it supposed to save in the lab) and puts the data for the record.Like this:men-.....,women-....,age 10-14-...,15-18-.... and so on.So, with this form you can get a picture.Sorry to dissappoint you, but the real number is at least twice bigger.Still, it is better than nothing.
