I read the Hungarian one and I am trying to explain a little bit. All these transdermal painkillers are usually for cancer and Parkinson patients (during my therapy I personally used plaster for very short time as the Contramal tablets and the contramal liquid (pump dose) provoked such strong vomiting that I could not take them. To avoid any drug addiction, the dose shall be very carefully set. The transdermal dose shall be calculated on the basis of morphin dose.

Switching from other opioids
When changing over from oral or parenteral opioids to fentanyl treatment, the initial dosage
should be calculated as follows:
1. The quantity of analgesics required over the last 24 hours should be determined.
2. The obtained sum should be converted to correspond to the oral morphine dosage using

3. The corresponding fentanyl dosage should be determined as follows:

The fentanyl patch is indicated for the management of persistent, moderate to severe chronic pain in opioid-tolerant patients 2 years of age or older who require a total daily opioid dose at least equivalent to fentanyl transdermal system 25 mcg/h. Opioid-tolerant patients are those who have been taking daily, for a week or longer, at least 60 mg of oral morphine, 30 mg of oral oxycodone, or at least 8 mg of oral hydromorphone or an equianalgesic dose of another opioid.
