This is the website of an ophthalmologist and the term appears in a list of *OCT images of the central part of the retina* (OCT Aufnahmen der Retinamitte).

Googling the term provided no results at all. I might be tempted to omit *durchgreifend*, except for the fact that the list also includes the term *Makulaloch durch Glaskörperzug* (which I translated as "macular hole caused by vitreous body traction").

Impending Macular Hole with Vitreomacular Traction (VMT)

Might be an idea for the translation of your second term (though I am not sure about the "impending"; you might want to do some research for that or leave the "impending" out, especially as patient language is required.

Thanks again, Susanne! I will look into it. As for patient language - I agree, but even the source is highly technical, so I won't worry too much about that.
