English to Italian: Werfarin Use and Outcomes in Patients with Atrial Fibrillation Complicating Acute Coronary Syndromes General field: Medical Detailed field: Medical (general)

Of the 23,208 patients enrolled in the PURSUIT, PARAGON-A, and SYNERGY trials, 5.8% (1346 patients) had atrial fibrillation as an in-hospital complication and 4.0% (917 patients) had atrial fibrillation and were discharged alive. Data on long-term outcomes were complete in 97% of the patients.
Baseline Characteristics According to Warfarin Therapy
Patients who received warfarin at discharge more often had a history of heart failure, diabetes, myocardial infarction, and stroke and less often had a history of hypertension when compared with patients who did not receive warfarin at discharge (Table 1). Otherwise, the baseline characteristics were similar between the 2 groups.
Medications at Discharge and In-hospital Procedures According to Warfarin Therapy
The discharge medications and in-hospital procedures according to warfarin use are shown in Table 2. Compared with patients who did not receive warfarin at discharge, patients who did receive warfarin at discharge were more likely to be discharged with an antiarrhythmic drug, a calcium channel blocker, an angiotensin-converting enzyme inhibitor, and a nitrate, and less likely to receive aspirin and a beta-blocker.
Warfarin Use According to Congestive Heart Failure, Hypertension, Age>75 Years, Diabetes, Prior Stroke/Transient Ischemic Attack Score and Bleeding Risk
Warfarin use among patients with atrial fibrillation was similar across CHADS2 scores: 0, 13%; 1, 14%;_2, 13%
(Figure 1). Among patients with atrial fibrillation, aspirin and ticlopidine/clopidogrel use rates also were similar
across CHADS2 scores. Of the components of the CHADS2 score, only prior heart failure had a statistically significant association with warfarin use at discharge (odds ratio 1.5; 95% confidence interval (CI), 1.02-2.28; P_.04). The use of warfarin according to bleeding risk is shown in Figure 2. Warfarin was used similarly and approximately 12% of the time, regardless of the risk of bleeding. Notably, as the risk of bleeding increased, the CHADS2 score followed the same pattern that illustrates the overlap of clinical factors associated with the risk of both stroke and bleeding. Patients at high risk of bleeding had a median CHADS2 score of 4, and 11.1% received warfarin; the rate of warfarin use was similar (11.9%) for patients at low risk of bleeding, and the median CHADS2 score was 1. Despite the increase in the median CHADS2 score, the use of aspirin and ticlopidine/ clopidogrel decreased as the risk of bleeding increased (Figure 2). Although patients who had prior stroke were more likely to receive warfarin at discharge (20%) than patients without a previous stroke (13%), the use of warfarin was low in both groups (Figure 3). Patients with a prior stroke were less likely to receive aspirin or ticlopidine/clopidogrel at discharge.
Six-Month Death or Myocardial Infarction
Factors associated with 6-month death or myocardial infarction are shown in Table 3. Warfarin use at discharge was strongly associated with a lower risk of death or myocardial infarction within 6 months (adjusted hazard ratio 0.39; 95% CI, 0.15-0.98; P_.04) (Figure 4). In the sensitivity analysis, after adjusting for the propensity of receiving warfarin at discharge, similar results were found; warfarin use at discharge was significantly associated with lower rates of the 6-month outcome of death or myocardial infarction (hazard ratio 0.38; 95% CI, 0.15-0.97; P_.04).
Other In-hospital Complications According to Warfarin Use at Discharge
Compared with patients who did not receive warfarin at discharge, patients who did had less in-hospital severe
bleeding (0.8% vs 2.5%), less ventricular fibrillation (24.2% vs 36.6%), less sustained ventricular tachycardia (25.8% vs 35.9%), more in-hospital stroke (3.2% vs 1.4%), more congestive heart failure (21% vs 15.6%), more sustained hypotension (15% vs 12%), more cardiogenic shock (6.5% vs 2.5%), and more transient ischemic attack (2.5% vs 0.5%).
CONCLUSIONS
Warfarin is associated with better 6-month outcomes among patients with atrial fibrillation complicating an acute coronary syndrome, but its use is infrequent and not related to stroke or bleeding risks. Among the components of the CHADS2 score, the only significant predictor of warfarin use at discharge was heart failure. CHADS2 score_2 was independently associated with worse outcomes. Antiplatelet agents, particularly ticlopidine/clopidogrel, were less often prescribed in patients at higher risk of bleeding. This study highlights the need to better understand optimal antithrombotic therapy for atrial fibrillation occurring in the setting of an acute coronary syndrome.

Dei 23.208 pazienti partecipanti agli studi PURSUIT, PARAGON-A e SYNERGY, il 5,8% (1.346 pazienti) ha sviluppato la fibrillazione atriale (FA) come complicanza intra-ospedaliera e il 4,0% (917 pazienti) sviluppante la FA è stato dimesso ancora in vita. I dati sui risultati a lungo termine si sono dimostrati completi nel 97% dei pazienti.
Caratteristiche di Fondo della Terapia con Warfarin
I pazienti che hanno ricevuto il warfarin alla dimissione ospedaliera avevano alle spalle, il più delle volte, una storia d’insufficienza cardiaca, diabete, infarto del miocardio e ictus; mentre, in minor numero, avevano sofferto di ipertensione (dati confrontati con pazienti che non avevano ricevuto il warfarin alla dimissione - Tavola 1). Per il resto, le caratteristiche di fondo sono, altrimenti, risultate simili tra i due gruppi.
Trattamenti alla Dimissione e Procedure Intra-Ospedaliere Conformi alla Terapia con Warfarin
Il trattamento alla dimissione e le procedure intra-ospedaliere conformi all’uso del warfarin sono indicati nella Tavola 2. In confronto ai pazienti che non avevano ricevuto il warfarin alla dimissione, i pazienti che, invece, l’avevano ricevuto, hanno avuto più possibilità di essere dimessi con farmaci antiaritmici, calcio-antagonisti, ACE-inibitori e nitrati, piuttosto che ricevere aspirina e beta-bloccanti.
Uso del Warfarin rispetto a Insufficienza Cardiaca Congestizia, Ipertensione, Età Superiore ai 75 Anni, Diabete, Ictus Pregresso/Attacco Ischemico Transitorio e Rischio di Sanguinamento
L’uso del warfarin tra i pazienti con FA è stato simile relativamente al punteggio CHADS₂: 0, 13%; 1, 14%; ≥2, 13% (Figura 1). Tra i pazienti affetti da FA, anche i tassi di utilizzo di aspirina e ticlopidina/clopidogrel sono stati simili relativamente al punteggio CHADS₂. Tra gli elementi del punteggio CHADS₂, solo una precedente insufficienza cardiaca ha avuto un’associazione statisticamente rilevante con l’utilizzo del warfarin alla dimissione (OR 1.5; 95% intervallo di confidenza (CI) del 95%, 1.02-2.28; P= .04).L’uso del warfarin rispetto al rischio di sanguinamento è mostrato nella figura 2. Il warfarin è stato utilizzato similmente circa il 12% delle volte malgrado il rischio di sanguinamento. In particolare, all’aumento del rischio di sanguinamento, il punteggio CHADS₂ ha seguito lo stesso schema che illustra la sovrapposizione dei fattori clinici associati ai rischi di ictus e sanguinamento. I pazienti ad alto rischio di sanguinamento avevano un punteggio CHADS₂ medio di 4, e l’11,1% ha ricevuto il warfarin; il tasso di utilizzo del warfarin è stato similare (11,9%) nei pazienti a basso rischio di sanguinamento e, in quel caso, il punteggio CHADS₂ è stato di 1. Nonostante l’aumento nel punteggio CHADS₂ medio, l’uso di aspirina e ticlopidina/clopidogrel è diminuito all’aumentare del rischio di sanguinamento (Figura 2). Anche se i pazienti con ictus pregresso avevano più probabilità di ricevere il warfarin alla dimissione (20%) rispetto ai pazienti che non ne hanno sofferto (13%), l’uso del warfarin è rimasto basso in entrambi i gruppi (Figura 3). I pazienti con ictus pregresso avevano meno probabilità di ricevere aspirina o ticlopidina/clopidogrel alla dimissione ospedaliera.
Morte o Infarto Miocardico a Sei Mesi
I fattori associati a morte o a infarto miocardico a 6 mesi sono indicati nella Tavola 3. L’uso del warfarin alla dimissione è stato fortemente associato a un rischio più basso di morte o infarto del miocardio a 6 mesi (aggiustato per il rapporto di rischio a 0.39; CI del 95%, 0.15-0.98; P= .04) (Figura 4). Nell’analisi di sensitività, dopo aver aggiustato la tendenza a ricevere il warfarin alla dimissione, sono stati trovati risultati simili. L’uso del warfarin alla dimissione è stato prevalentemente associato a tassi inferiori a 6 mesi per morte o infarto del miocardio (rapporto di rischio a 0.38; CI del 95%, 0.15-0.97; P= .04).
Altre Complicazioni Intra-Ospedaliere Riguardanti l’Uso del Warfarin alla Dimissione
A confronto con pazienti che non hanno ricevuto il warfarin alla dimissione, i pazienti che l’hanno ricevuto hanno avuto meno casi intra-ospedalieri di sanguinamento grave (0,8% contro 2,5%), fibrillazione ventricolare (24,2% contro 36,6%), tachicardia ventricolare sostenuta (25,8% contro 35,9%), più casi intra-ospedalieri di ictus (3,2% contro 1,4%), d’insufficienza cardiaca congestizia (21% contro 15,6%), ipotensione sostenuta (15% contro 12%), shock cardiogeno (6,5% contro 2,5%) e più attacchi ischemici transitori (2,5% contro 0,5%).
CONCLUSIONI
Il warfarin è associato a migliori risultati a sei mesi tra i pazienti con fibrillazione atriale complicante una sindrome coronarica acuta ma, il suo uso non è frequente e non è connesso a rischi di ictus e sanguinamento. Tra gli elementi del punteggio CHADS₂, l’unico predittore indicativo dell’uso del warfarin alla dimissione è stato l’insufficienza cardiaca. Il punteggio CHADS₂ ≥2 è stato indipendentemente associato a risultati peggiori. Gli agenti anti-piastrine, in particolare ticolpidina/clopidogrel, sono stati prescritti, in numero di volte minore, ai pazienti con un più alto rischio di sanguinamento. Questo studio sottolinea la necessità di conoscere al meglio la terapia antitrombotica ottimale per la fibrillazione atriale ricorrente durante una sindrome coronarica acuta.
