Le virus B du singe ou Cercopithecine herpesvirus 1 (CeHV-1) appartient à la vaste famille Herpesviridae et plus précisément au genre Simplexvirus de la sous-famille des Alphaherpesvirinae [1] (figure 1). Ce virus proche de l’herpèsvirus humain 1 (human herpesvirus 1, HHV-1 ; ou herpes simplex virus, HSV-1), agent causal de l’herpès labial chez l’homme et de l’HHV-2 ou HSV-2, agent de l’herpès génital [1], est endémique chez les singes du genre Macaca [2, 3]. Il présente en outre la particularité d’être le seul herpèsvirus de primates non humains à être pathogène pour l’homme [2, 3]. Une meilleure connaissance de la biologie de ce virus ainsi que l’adoption de mesures préventives strictes et adaptées sont cruciales pour protéger les personnes en contact avec des singes infectés ou avec des tissus et cellules provenant de ces primates [4, 5]. Cet article abordera successivement l’historique de l’infection, le virus, sa pathogénie, la symptomatologie de l’infection, l’hôte, le diagnostic clinique et de laboratoire, les mesures de prévention et de traitement de l’anthropozoonose ainsi qu’une observation d’un cas clinique et son traitement sur le terrain en Afrique.
Le Cercopithecine herpesvirus 1 appartient au genre Simplexvirus de la famille des Alphaherpesvirinae.
Historique et virus
La première description connue de l’anthropozoonose associée au virus B remonte au début des années 1930 et plus exactement à l’année 1932 qui vit le docteur W.B. succomber suite à une morsure de macaque rhésus (Macaca mulatta) [5]. L’agent responsable, filtrable, fut identifié, d’abord par Gay et Holden en 1933, ensuite par Sabin et Wright, en 1934, qui le nommèrent virus B en hommage à la première victime [6, 7]. La dénomination vernaculaire du CeHV-1, « virus B », est utilisée dans cet article. Toutefois, il est à garder à l’esprit que beaucoup d’autres appellations existent, notamment : Herpesvirus simiae, herpès B, monkey B virus, herpesvirus B [5]. Les herpèsvirus sont des virus enveloppés à ADN bicaténaire linéaire [1]. La particule virale avoisine les 200 nm de diamètre et possède une nucléocapside de symétrie icosaédrique [5]. Le génome du virus B présente une taille de 157 kbp et les 74 gènes identifiés codent pour des protéines homologues, à une exception près, à celles de l’HHV-1 [8].
Pathogénie
Chez l’homme, cette maladie est une zoonose majeure avec un mode de transmission direct par morsure, griffure ou aérosol [9, 10]. Les infections par le virus B se caractérisent par une implication importante du système nerveux central et plus particulièrement de la moelle épinière [5]. En effet, le virus présente un neurotropisme et une neurovirulence marqués menant à une issue fatale dans 80 % des cas en l’absence de traitement [3, 5]. Un traitement antiviral précoce et adapté permet de réduire considérablement la morbidité et la mortalité humaine [4]. La prévalence de l’excrétion virale est augmentée lorsque les singes sont stressés, gestants, immunodéprimés ou malades. Près de 100 % des macaques captifs d’âge supérieur à 2,5 ans et environ 20 % d’âge inférieur à 2,5 ans sont séropositifs au virus B [4]. Par ailleurs, de 1/50 à 1/250 contacts avec des macaques aboutissent à une exposition potentielle à des substances contaminées par le virus B [4]. Les morsures à la tête et au cou sont les plus susceptibles d’aboutir à une maladie fatale alors que les morsures aux doigts ou aux mains sont moins graves [4]. En général, les alphaherpèsvirus infectent la muqueuse épithéliale, effectuent un ou plusieurs cycles de multiplication virale dans les cellules épithéliales et ensuite après lyse cellulaire pénètrent dans d’autres cellules et les terminaisons nerveuses [1, 5]. Ensuite, le génome viral persiste à l’état latent dans les ganglions sensitifs [1, 5]. Cette latence est caractérisée par l’absence de la réplication virale et la limitation de la transcription [1, 5]. Une grande charge virale dans les ganglions sensitifs augmente la fréquence de la réactivation et de l’excrétion du virus [10, 11]. Une simple rupture de la barrière cutanée en présence du virus ou l’instillation de ce même virus au niveau d’une muqueuse peut aboutir au déclenchement de la maladie suggérant que la capacité du virus à initier la maladie pourrait être indépendante de la dose, du moins dans certaines circonstances [10, 11]. Néanmoins, la quantité de virus nécessaire ou suffisante pour provoquer l’infection reste inconnue chez l’homme [10, 11].
Symptomatologie
Chez le singe
Le virus B peut être à l’origine de gingivite, de stomatite, d’ulcères linguaux, oraux et génitaux et de conjonctivite [10, 12, 13]. Toutefois, ces lésions sont le plus généralement décrites chez des animaux stressés ou immunodéprimés et l’infection reste le plus souvent asymptomatique [10, 12, 13]. L’animal est généralement infecté par exposition de ses muqueuses ou de sa peau aux secrétions orales ou génitales de ces congénères [3]. La transmission verticale du virus est rare étant donné l’incapacité du virus à traverser la barrière placentaire. Après multiplication au site initial d’infection, le virus persiste au niveau des ganglions nerveux sensitifs et des terminaisons nerveuses pendant toute la vie de l’animal et peut se réactiver avec réexcrétion virale au niveau des muqueuses orales, conjonctivales ou génitales, sans aucune lésion macroscopique [3, 4].
Chez l’homme
La transmission interhumaine a été décrite à une reprise, cependant les investigations ultérieures ont indiqué que le risque de la transmission secondaire était faible [3, 4]. Une bonne connaissance du tableau clinique associé à la maladie est importante pour fournir un diagnostic rapide des infections à virus B chez l’homme. La reconnaissance rapide de la maladie augmente considérablement le taux de succès du traitement basé sur l’utilisation de substances antivirales [4, 9]. Les humains exposés au virus B présentent des signes cliniques variables [3, 5, 11, 14]. Le plus souvent, la maladie apparaît quelques jours à quelques semaines après l’exposition et dure de 1 à 3 semaines avec une issue mortelle dans 80 % des cas en l’absence de traitement [3, 5, 11, 14]. Cependant, la maladie peut inexplicablement apparaître plus tardivement. Dans un premier temps, la zone d’inoculation du virus devient congestive, douloureuse et prurigineuse [3, 5, 11, 14]. Des lésions vésiculeuses autour de la plaie, une lymphadénopathie régionale et une légère fièvre sont également constatées. Par la suite, l’apparition d’un syndrome pseudo-grippal accompagné de maux de gorge est observée [3, 5, 11, 14]. En outre, le patient peut signaler des paresthésies et des fourmillements à partir du site d’exposition, ainsi qu’une faiblesse musculaire au niveau de l’extrémité exposée. Ensuite, survient un syndrome méningoencéphalitique, accompagné de douleurs musculaires, céphalée, raideur et vomissements. L’évolution se fait vers l’ataxie, la paralysie et finalement la mort par défaillance respiratoire. Des signes d’atteinte des nerfs crâniens sont également présents, tel du nystagmus et de la diplopie. De la sinusite et de la conjonctivite ont également été décrites [3, 5, 11, 14].
Le singe
Le macaque Rhésus constitue le principal réservoir du virus ; toutefois, d’autres espèces de primates sont également sensibles à des infections zoonotiques [3, 5, 11, 15-17]. Ainsi, le genre Macaca, plus utilisé dans les recherches biomédicales, est le plus communément touché (tableau 1). Toutefois, le cas clinique rapporté dans cet article rappelle que le genre Cercopithecus est aussi vecteur du virus. L’infection par le virus B a été décrite chez des singes non-macaques : Erythrocebus patos (singe rouge) ; Colobus guereza (guéreza du Kilimandjaro) ; Cebus paella (singe capuchin) ; Cebus paella (singe capucin brun) ; Callithrix jacchus (marmouset commun) ; Cercopithecus neglectus (cercopithèque de Brazza). Dans le cas clinique décrit ci-dessous, le singe suspect de l’infection est un singe grivet (Cercopithecus aethiops) (figure 2). Ce singe mesure de 40 à 60 cm de la tête à l’arrièretrain avec une queue de 42-70 cm de long. Son poids total varie de 5 à 8 kg. Le dos est gris, le ventre et les faces internes des membres sont blanchâtres, la face est noire (bordée de moustaches blanches) et l’extrémité des membres est gris pâle (figure 2). Il habite la savane, les clairières et les espaces cultivés où il se nourrit de feuilles d’acacia, de fleurs, de graines, de jeunes pousses et de fruits [18].
Diagnostic
Chez le singe
À ce jour, peu d’outils sérologiques permettent d’identifier de manière formelle les animaux infectés [5, 19]. En effet, la parenté antigénique importante entre le virus B et d’autres herpèsvirus complique singulièrement le diagnostic tout en n’offrant pas de protection croisée [20-22]. L’isolement viral devrait constituer par conséquent une méthode de choix pour le diagnostic de l’infection au virus B. Malheureusement, cet outil n’est pas particulièrement sensible et le risque d’obtention de résultats faussement négatifs persiste. De plus, la culture du virus nécessite l’utilisation de structures lourdes présentant un niveau de sécurité biologique élevé [5]. Plus récemment, afin de pallier ces problèmes, plusieurs tests PCR ont été développés [5, 23-26]. Chez bon nombre d’espèces de singes, l’aspect diagnostic important sera de différencier les anticorps spécifiques du virus B des anticorps spécifiques d’autres virus, tel par exemple l’HHV-1 [27]. Sur l’animal mort, le diagnostic se fait par examen histologique des lésions des muqueuses buccale, linguale, oesophagienne et gastrique.
Chez l’homme
Le virus B est classé au niveau 4 de sécurité biologique ; il appartient ainsi au même groupe de risques que les virus Ebola et de Marburg. Lorsqu’une infection par le virus B est suspectée, la méthode ultime pour confirmer le diagnostic consiste en l’isolement viral [5]. Lorsque celui-ci n’est pas envisageable, la PCR peut être utilisée [5, 23]. Le diagnostic sérologique est rendu complexe par un certain nombre d’éléments : tout d’abord parce que les anticorps anti- HHV-1, HHV-2 (incapables de neutraliser le virus B) et anti-virus B (incapables de neutraliser les HHV-1 et -2) sont proches, ensuite parce que les titres en anticorps sont parfois faibles [3].
Mesures de prévention de l’infection
Le personnel travaillant avec des macaques doit impérativement être informé du risque encouru [4, 9]. Les premiers symptômes de l’infection ainsi que la conduite à tenir en cas d’exposition doivent être connus du personnel. Toute morsure de primate devra être traitée avec prudence par un médecin de référence [4, 9]. De plus, tout animal n’ayant pas été reconnu séronégatif pour le virus B doit être considéré comme infecté et par conséquent manipulé avec précaution et tout animal reconnu infecté, porteur sain ou malade, devra être euthanasié [4, 9]. Les premières minutes constituent la période critique de la prévention de l’infection. Les yeux et les muqueuses, potentiellement exposés au virus, doivent être irrigués immédiatement au moyen d’une solution saline stérile ou, si ce n’est possible, avec de l’eau courante pendant 15 minutes. La peau potentiellement exposée doit être nettoyée à la Povidone iodine, chlorhexidine ou au savon détergent ; ces solutions détruisent l’enveloppe du virus et peuvent par conséquent l’inactiver [4, 9]. Le protocole de prévention de l’infection humaine recommande de procéder dans les 2 à 3 premières minutes qui suivent l’accident à : – un lavage immédiat du site d’inoculation à grande eau et au savon de Marseille pendant 3 à 5 minutes ; pour les plaies profondes, faire saigner sous le robinet ; – désinfection par trempage de la plaie pendant 10 minutes avec de l’eau de Javel 12° diluée 10 fois ; – en cas de blessures par projection oculaire, remplacer ces soins par un rinçage à l’eau courante pendant 10 à 15 minutes. Toute lésion cutanée causée par un macaque ou une cage potentiellement contaminée par les secrétions de macaques, doit être immédiatement et abondamment nettoyée à l’eau et au savon. Un traitement préventif avec les antiviraux doit être entrepris quand les blessures sont profondes [4, 9].
Vaccination et traitement
Dès les années 1930, des tentatives de production d’un vaccin ont été menées [17]. Des tests ont été réalisés sur des volontaires humains mais n’ont guère été concluants : anticorps non persistants et titres trop faibles. Actuellement, des vaccins recombinants et des vaccins à base d’ADN sont testés avec des résultats préliminaires encourageants [3, 28]. En matière de traitement, c’est essentiellement l’acycloguanosine appelée acyclovir (ACV ou Zovirax®) qui est utilisée. L’ACV est administré selon les cas par perfusion intraveineuse (formes graves), per os (pour les affections moins sévères) ou en application oculaire dans les cas de kératoconjonctivite [3, 4, 9]. L’ACV, en tant qu’inhibiteur de la réplication de l’ADN viral n’a pas d’action sur les virus latents dans les ganglions sensitifs puisque leur ADN ne se réplique pas. Ainsi l’ACV n’éradique pas l’infection, de sorte que les poussées peuvent revenir à l’arrêt du traitement. Le valacyclovir (Zélitrex®), converti rapidement en ACV, a une biodisponibilité 5 fois supérieure à celle de l’ACV. Le traitement antiviral a démontré son efficacité, particulièrement lorsque les substances actives sont administrées tôt après l’exposition [3, 4, 9]. L’ACV et les analogues nucléosidiques apparentés à fortes doses (10 mg/kg par voie intraveineuse, 3 fois par jour durant 14 à 21 jours) suivis d’un traitement per os (acyclovir, valacyclovir ou famcyclovir) à vie donnent de bons résultats [3, 4, 9]. Le traitement est donc idéalement instauré à vie, sans interruption étant donné les risques de reprise évolutive de la maladie après réactivation virale. D’autres substances se sont montrées efficaces in vitro, toutefois leur toxicité doit encore être évaluée [5].
Observation d’un cas en République démocratique du Congo
M, de sexe féminin, âgée de 30 ans, est mordue le soir du 23 avril 2007 au Parc national de la Garamba, par une guenon cercopithèque stressée car elle venait d’être récupérée des mains de son ancien gardien. Le matin du 24 avril, elle consulte le Centre de Santé de Nagero pour plaies, douleur et gonflement à la main droite. À l’examen physique, trois plaies sont décrites : deux plaies linéaires à bords nets, d’environ 1 cm de long sur 0,5 cm de profondeur et une plaie ovoïde à bords irréguliers d’environ 1 cm de diamètre. Les régions touchées sont congestionnées et la main droite présente une tuméfaction. Le diagnostic final n’étant pas encore connu, la blessée a été placée sous sérothérapie antitétanique, a bénéficié d’une désinfection profonde des plaies et d’un traitement symptomatique de la douleur et de la tuméfaction de la main par le Diclofénac à raison de 3 × 50 mg/jour per os. Deux jours plus tard, la patiente présente de la fièvre avec une température de 38,6 °C ainsi qu’un syndrome pseudo-grippal. Après une rémission apparente d’environ deux semaines, elle montre le 15 mai un gonflement de l’avant-bras droit accompagné de fourmillements, de faiblesse musculaire et des petites masses axillaires droites. À l’examen physique, la cicatrisation des plaies est constatée ainsi qu’une tuméfaction intéressant la main et l’avantbras droits et des adénopathies axillaires droites sensibles, libres par rapport aux plans superficiel et profond. Après une deuxième rémission de deux semaines, la patiente se plaint le 30 mai de sensations de prurit aux sites des morsures, de tuméfaction et de douleurs profondes intéressant la main et l’avant-bras droits. La réapparition, le 5 juin, de mêmes signes cliniques que la semaine précédente, pousse l’équipe médicale, après documentation, à une forte suspicion d’infection par le virus B. Étant donné les moyens limités et l’impossibilité de réalisation des tests de laboratoire localement, le diagnostic d’herpèsvirose B est posé sur une base clinique uniquement en se basant sur le contexte d’exposition (morsure par un cercopithèque stressé), les symptômes typiques ainsi que l’évolution clinique de la patiente avant, pendant et après traitement à l’ACV. Le traitement à l’ACV, promptement mis en place, est administré per os à la dose de 4 × 1 comprimé/jour (200 mg) en respectant l’intervalle de 6 heures entre les prises. Les singes en captivité dans les familles à Nagero sont isolés et ensuite euthanasiés. La périodicité de réapparition des signes cliniques est décrite à cause des accès de réactivation de cette infection à herpèsvirus. Au cours du traitement, la patiente s’est plainte de céphalées, de fatigue et d’insomnie. Ces symptômes, considérés comme effets secondaires du traitement à l’ACV, ont été soulagés au moyen de paracétamol, au besoin, à la dose de 2 × 1 g/jour. Deux semaines après le début du traitement, la patiente a présenté une douleur, une tuméfaction et une rougeur limitées au pouce de la main droite sans cause apparente. Après examen physique, on a conclu à une récidive clinique par réactivation virale, la patiente a été rassurée et le traitement poursuivi. Deux jours plus tard, les symptômes avaient disparu. La patiente a pris au total une centaine de comprimés d’ACV 200 mg sans interruption pendant une durée totale de 25 jours. Les structures locales étant souséquipées, il n’a guère été possible d’apprécier biologiquement l’efficacité du traitement. Néanmoins, cliniquement et en tenant compte des limites inhérentes au terrain, le traitement semble constituer un succès. Toutefois, en prévision de réactivations ultérieures hélas fort probables, du fait de la latence virale, la patiente reste sous surveillance médicale. Actuellement, plus de 11 mois après l’arrêt du traitement et quasiment une année après la dernière manifestation clinique typique, la patiente ne présente pas de troubles.
Conclusion
Dans les milieux isolés et sous-équipés, le diagnostic clinique est particulièrement important. En Afrique centrale, son importance est encore accentuée par les nombreuses possibilités de contact homme/singe aussi bien en milieu rural qu’urbain. Sur place, les singes se retrouvent en captivité comme animal de compagnie, sont consommés pour la viande et déambulent aux alentours des habitations en quête de nourriture. En conclusion, l’infection humaine par le virus B peut conduire à une maladie rapidement mortelle. Toutefois, lorsque le diagnostic est posé suffisamment tôt et que le traitement antiviral adéquat est entrepris promptement, la probabilité de guérison du patient est élevée. Les recommandations relatives au traitement et à la prévention de la maladie devront être disponibles et connues par toute personne en contact avec les singes. Les nouvelles méthodes de diagnostic améliorent le seuil de détection de l’infection virale. La vaccination nécessite de nouveaux développements. Dès lors, et étant donné les risques pour la santé humaine occasionnés par cet alphaherpèsvirus, des précautions drastiques doivent être prises. Ainsi, une attention particulière sera portée notamment au logement et à la gestion des singes captifs ainsi que sur la conduite à suivre après exposition humaine à un animal ou à du matériel contaminé. ■

Simian herpes B virus, or Cercopithecine herpesvirus 1 (CeHV-1), belongs to the vast family Herpesviridae: to the genus Simplexvirus of the subfamily Alphaherpesvirinae (fig 1).[1] This virus is similar to Human herpesvirus 1 (HHV-1, also known as herpes simplex virus 1 or HSV-1), the causative agent of herpes of the lips in man, as well as to HHV-2 (or HSV-2), which causes genital herpes.[1] It is endemic among monkeys of the genus Macaca.[2, 3] It is also noteworthy for being the only herpesvirus of non-human primates that can cause illness in man.[2, 3] A better knowledge of the biology of this virus is crucial for the protection of people coming into contact with infected monkeys or with tissues or cells originating from them, as is the adoption of strict, tailored preventive measures.[4, 5] This article deals in turn with the history of the infection, the virus, its pathogenesis, the symptoms associated with infection, the host, clinical and laboratory diagnosis and measures for preventing and treating this anthropozoonosis, as well as observation of a clinical case and its treatment on the ground in Africa.
Cercopithecine herpesvirus 1 belongs to the genus Simplexvirus of the subfamily Alphaherpesvirinae.
The history and the virus
The first known description of the anthropozoonosis linked to B virus appeared at the start of the 1930s. 1932 saw Dr WB succumb to a bite from a rhesus macaque (Macaca mulatta).[5] The filterable agent responsible was first identified by Gay and Holden in 1933, and then by Sabin and Wright in 1934, who named it B virus as a tribute to its first victim.[6, 7] This is the vernacular term we have chosen to use for CeHV-1 in this article. However, it should be borne in mind that many other names exist, notably herpesvirus simiae, herpes B, monkey B virus, and herpesvirus B.[5] The herpesviruses are enveloped, with double-stranded linear DNA.[1] The viral particle is nearly 200 nm in diameter and has a nucleocapsid with icosahedral symmetry.[5] The genome of B virus is 157 kbp in size and 73 of the 74 genes that have been identified code for proteins homologous to those of HHV-1.[8]
Pathogenesis
In man, this disease is a major zoonosis with a direct mode of transmission, i.e. through bites or scratches or by aerosol transmission.[9, 10] Infections with B virus are characterised by extensive involvement of the central nervous system, particularly of the spinal cord.[5] Indeed, the virus demonstrates marked neurotropism and neurovirulence, leading to a fatal outcome in 80% of cases in the absence of treatment.[3, 5] Early, tailored antiviral treatment considerably reduces human morbidity and mortality.[4] The prevalence of viral excretion increases when the monkeys are stressed, pregnant, immunodepressed or ill. Nearly 100% of captive macaques over 2.5 years old and around 20% of those below that age are seropositive for B virus.[4] In addition, between 1 in 50 and 1 in 250 contacts with macaques lead to potential exposure to substances contaminated with B virus.[4] Bites to the head and neck are more likely to lead to fatal disease, while bites to the fingers or hands are less serious.[4] In general, the alphaherpesviruses infect the epithelial mucus, going through one or several cycles of viral multiplication in the epithelial cells and then, after cell lysis, penetrating other cells and the nerve endings.[1, 5] Then the viral genome persists in a latent state in the sensory ganglions.[1, 5] This latency is characterised by the absence of viral replication and by limited transcription.[1, 5] A high viral load in the sensory ganglions increases the frequency of reactivation and excretion of the virus.[10, 11] A mere rupture of the skin barrier in the presence of this virus or instillation of the virus into mucous membrane can lead to the onset of the disease, which suggests that the virus’s capacity to initiate the disease might be independent of the dose: at least under certain circumstances.[10, 11] Nevertheless, the quantity of the virus that is necessary or sufficient to initiate the infection remains unknown in man.[10, 11]
Symptomatology
In monkeys
B virus may cause gingivitis, stomatitis, ulcers of the tongue, mouth and genitals, and conjunctivitis.[10, 12, 13] However, these lesions are most frequently described in stressed or immunodepressed animals, and the infection usually remains asymptomatic.[10, 12, 13] The animal is generally infected by the exposure of its mucosa or skin to the oral or genital secretions of its fellows.[3] Vertical transmission of the virus is rare given the virus’s inability to cross the placental barrier. After multiplication at the initial site of infection, the virus persists in the sensory nerve ganglions and nerve endings throughout the animal’s life, and may be reactivated, with viral excretion in the oral, conjunctival or genital mucus, without any macroscopic lesions.[3, 4]
In man
Human-to-human transmission has been described on one occasion, though subsequent investigations have indicated that the risk of secondary transmission was low.[3, 4] Good knowledge of the clinical picture associated with the disease is important for the rapid diagnosis of B virus infections in man. Rapid recognition of the disease considerably increases the success rate of treatment based on the use of antiviral substances.[4, 9] Humans exposed to B virus present with variable clinical symptoms.[3, 5, 11, 14] The disease usually appears from a few days to a few weeks after exposure and lasts from one to three weeks, with a fatal outcome in 80% of cases in the absence of treatment.[3, 5, 11, 14] However, inexplicably, the disease may appear later. Initially, the site of inoculation of the virus becomes congested, painful and pruritic.[3, 5, 11, 14] Vesicular lesions around the wound, localised lymphadenopathy and a slight fever have also been reported. Later, flu-like symptoms appear, accompanied by a sore throat.[3, 5, 11, 14] In addition, the patient may report paresthesias and tingling originating from the site of exposure, as well as muscle weakness in the affected extremity. Then comes meningismus accompanied by muscular pain, headaches, stiffness and vomiting. This develops into ataxia, paralysis and finally death by respiratory failure. There are also signs that the cranial nerves are affected, i.e. nystagmus and diplopia. Sinusitis and conjunctivitis have also been observed.[3, 5, 11, 14]
The monkey
The rhesus macaque is the main reservoir of the virus. However, other species of primates are also prone to zoonotic infections.[3, 5, 11, 15-17] The genus Macaca, which is the most used in biomedical research, is the one most commonly affected in this way (table 1). Nevertheless, the clinical case reported in this article reminds us that the genus Cercopithecus is also a vector of the virus. Infection by B virus has been described in the following non-macaque monkeys: Erythrocebus patas (patas monkey); Colobus guereza (guereza); Cebus paella (capuchin monkey); Cebus paella (brown capuchin); Callithrix jacchus (common marmoset); Cercopithecus neglectus (de Brazza’s monkey). In the clinical case described below, the monkey suspected of causing the infection is a vervet monkey (Cercopithecus aethiops) (fig 2). This monkey measures between 40 and 60 cm from head to rump, with a tail 42-70 cm in length. Its total weight varies from 5 to 8 kg. Its back is grey, its underside and the inner surfaces of its limbs are whitish, its face is black (surrounded by white whiskers), and the extremities of its limbs are pale grey (fig 2). It lives in the savannah, in clearings and in cultivated areas, where it feeds on acacia leaves, flowers, seeds, young shoots and fruit.[18]
Diagnosis
In monkeys
To date, there are few serological tools that permit the formal identification of infected animals.[5, 19] Indeed, the close antigenic relationship between B virus and other herpesviruses radically complicates diagnosis, but does not offer any cross-protection.[20-22] Therefore, virus isolation must constitute the method of choice for diagnosing infection with B virus. Unfortunately, this is not a particularly sensitive tool, and the risk of obtaining false-negative results remains. Moreover, culturing the virus requires the use of unwieldy procedures that offer a high level of biosafety.[5] More recently, in order to compensate for these problems, several PCR tests have been developed. [5, 23-26] In a good number of monkey species, the important diagnostic factor will be differentiating between the specific antibodies of B virus and the specific antibodies of other viruses, such as HHV-1, for example.[27] Once the animal is dead, the diagnosis can be made by histological examination of the lesions of the buccal, lingual, oesophageal and gastric mucosa.
In man
B virus is classified as Biosafety Level 4: this means that it belongs to the same risk group as Ebolavirus and Marburgvirus. If infection with B virus is suspected, the ultimate method for confirming the diagnosis consists in isolating the virus.[5] If this is not feasible, PCR may be used.[5, 23] Serological diagnosis is complicated by a number of factors: firstly, the close relationship between the anti-HHV-1 and anti-HHV-2 antibodies (incapable of neutralising B virus) and the anti-B virus (incapable of neutralising HHV-1 and -2), and secondly, the fact that the antibody titres are sometimes low.[3]
Measures for preventing infection
It is absolutely imperative that personnel working with macaques are informed of the risk they are running.[4, 9] They should be aware of the initial symptoms of infection, as well as what to do in the event of exposure to the virus. Any bites from a primate should be treated with care by a specialist.[4, 9] In addition, any animal that has not been identified as seronegative for B virus should be considered infected and therefore handled with care. Euthanasia is strongly advised for any animal that has been identified as infected, whether ill or a healthy carrier.[4, 9] The first few minutes are critical in preventing infection. The eyes and mucous membranes which may have been exposed to the virus must be irrigated immediately with a sterile saline solution – or, if this is not possible, under running water – for 15 minutes. Skin which may have been exposed must be washed with povidone iodine, chlorhexidine or detergent soap: these solutions destroy the viral envelope and can therefore inactivate it.[4, 9] The protocol for the prevention of human infection recommends that, within the first 2 to 3 minutes following the accident: (a) the inoculation site should be washed with soap (Savon de Marseille) and plenty of water for 3 to 5 minutes; (b) deep wounds should be cleaned under the tap; (c) the wound should be disinfected by immersion for 10 minutes in bleach diluted to 0.36% active chlorine; (d) in case of injury by bodily fluids splashing into the eye, this treatment should be replaced with rinsing under running water for 10 to 15 minutes. Any skin lesions caused by a macaque, or by a cage that could have been contaminated by the secretions of macaques, should be immediately cleaned with plenty of soap and water. Preventive treatment with antivirals must be undertaken if the wounds are deep.[4, 9]
Vaccination and treatment
Attempts have been made to produce a vaccine since the 1930s.[17] Tests carried out on human volunteers turned out inconclusive, with short-lived antibodies and titres that were too low. At present, recombinant and DNA-based vaccines are being tested, and the preliminary results are encouraging.[3, 28] Acycloguanosine, known as aciclovir (ACV or Zovirax®), is the treatment most commonly used. Depending on the case in question, ACV may be administered by intravenous infusion (for serious cases), orally (for less severe complaints), or into the eye in cases of keratoconjunctivitis.[3, 4, 9] ACV, which inhibits the replication of viral DNA, does not have any effect on latent viruses in the sensory ganglions, because their DNA is not replicating. Thus, ACV does not eradicate the infection, which means that relapses may occur upon the cessation of treatment. Valaciclovir (Zelitrex®), which is converted rapidly into ACV, has a bioavailability five times greater than that of ACV itself. Antiviral treatment has been shown to be effective, particularly when the active substances are administered soon after exposure.[3, 4, 9] Good results are obtained with high doses of ACV and related nucleoside analogues (10 mg/kg administered intravenously, three times a day for 14 to 21 days), followed by oral treatment (aciclovir, valaciclovir or famciclovir) for the rest of the patient’s life.[3, 4, 9] Thus, treatment should ideally be continued for life, and should not be interrupted given the risks of the disease returning with increased severity upon reactivation of the virus. Other substances have been shown to be effective in vitro, though their toxicity has yet to be evaluated.[5]
Observation of a case in the Democratic Republic of the Congo
On the evening of 23 April 2007, M, a 30-year-old woman, was bitten by a female cercopithecine monkey in the Garamba National Park. The animal was under stress, having just been removed from its former keeper. On the morning of 24 April, the woman consulted the health centre in Nagero complaining of cuts, pain and swelling in her right hand. Upon physical examination, three wounds were described: two clear-cut linear wounds, about 1 cm long by 0.5 cm deep, and one oval wound with irregular edges, about 1 cm in diameter. The affected areas were congested and there was swelling of the right hand. The final diagnosis being as yet unknown, the injured woman was treated with antitetanus serotherapy and underwent thorough disinfection of her wounds, as well as symptomatic treatment of the pain and swelling in her hand with oral diclofenac, 50 mg three times daily. Two days later, the patient presented with a fever of 38.6°C and flu-like symptoms. On 15 May, after a couple of weeks’ apparent remission, she presented with swelling of the right forearm accompanied by tingling, muscle weakness and small lumps in the right axilla. Upon physical examination, it was observed that the wounds had healed and that there was swelling of the right hand and forearm, as well as sensitive right axillary adenopathy, but both superficial and deep nodes were mobile. On 30 May, after a second two-week period of remission, the patient complained of pruritus around the bites, as well as swelling and acute pain in the right hand and forearm. On 5 June, the reappearance of the same clinical signs as in the previous week led the medical team, after some research, strongly to suspect infection by B virus. Given their limited facilities and the impossibility of carrying out laboratory tests locally, the diagnosis of herpesvirus B was made solely on a clinical basis, based on the context of exposure (bites from a cercopithecine monkey under stress), the typical symptoms, and the clinical progress of the patient before, during and after ACV treatment. ACV treatment was begun promptly and was administered orally, one tablet (200 mg) four times a day, with six-hour intervals between doses. The monkeys kept by families in Nagero were isolated and then put down. The pattern of recurrence of the clinical signs was ascribed to bouts of reactivation of the herpesvirus infection. During treatment, the patient complained of headaches, fatigue and insomnia. These symptoms, which were considered to be side-effects of the ACV treatment, were relieved by means of paracetamol as required, at a dose of one tablet (1g) twice a day. Two weeks after the start of treatment, the patient presented with pain, swelling and redness limited to the thumb of the right hand without any apparent cause. After a physical examination, it was concluded that she was having a clinical relapse due to viral reactivation, the patient was reassured and the treatment was continued. Two days later, the symptoms had disappeared. The patient had taken an approximate total of 100 200-mg ACV tablets for a total of 25 days, without interruption. As local facilities were underequipped, proper biological assessment of the effectiveness of the treatment had not been possible. Nevertheless, clinically, taking into account the inherent limitations on the ground, the treatment seemed to have been a success. However, since it must be anticipated that the infection is, unfortunately, very likely to reactivate at a later date due to viral latency, the patient is still under medical observation. At present, more than 11 months after the end of treatment and nearly a year after the last typical clinical manifestation, the patient is not showing any symptoms.
Conclusion
In isolated and under-equipped environments, clinical diagnosis is particularly important. In central Africa, its importance is further heightened by the numerous opportunities for contact between humans and monkeys, in both rural and urban areas. There, monkeys are found in captivity as pets, are consumed as meat and wander around the outskirts of residential areas in search of food. In conclusion, human infection by B virus can lead to a disease which rapidly becomes fatal. However, when it is diagnosed early enough and appropriate antiviral treatment is promptly undertaken, the likelihood of curing the patient is increased. All those in contact with monkeys must be made aware of the recommendations relating to treatment and prevention of the disease. New diagnostic methods are improving the detection threshold for infection with this virus. New developments will be required before a vaccine can be made available. Consequently, given the risks to human health posed by this alphaherpesvirus, special precautions must be taken. Thus particular attention must be paid to the accommodation and management of monkeys in captivity, as well as to the procedures that should be followed after human exposure to contaminated animals or material. ■

Drei Monate dauerte der Einsatz von Peter Ungemach in Bolivien. Dort sollte der Hamburger Diplom-Ingenieur, der zuletzt bei der Lufthansatechnik tätig war, Betriebsabläufe, Qualität und Sicherheit in einem privaten Luftverkehrsbetrieb mit 120 Mitarbeitern verbessern. Bei seiner Tätigkeit wies der Senior Experte unter anderem darauf hin, dass eine regelmäßige und strikt an den Vorgaben der Hersteller orientierte Wartung der Flugzeuge aufwändige und langwierige Reparaturen verhindern hilft. Außerdem sorgte er dafür, dass die Mitarbeiter, die bis zu 24 Stunden am Stück arbeiteten, durch eine langfristige Arbeitsplanung entlastet wurden. In punkto Arbeitssicherheit und Umweltschutz erarbeitete der Senior Experte zahlreiche Empfehlungen für die Firmenleitung. Die Kommunikation bei dem Einsatz verlief größtenteils in spanischer Sprache.
China: Neue Wege zum Öko-Tourismus
Übernachten in freier Natur, umgeben von Feldern, wandern entlang historisch bedeutender Stätten – Öko-Tourismus hat auch in China stark an Bedeutung gewonnen. Doch was ist touristisch anziehend und interessant? Wie können Urlauber aus dem eigenen Land geworben werden und wo sollen sie übernachten? Um diese Fragen zu klären, forderte die Pangu Teefabrik in Yudu im Südosten Chinas einen Senior Experten an. Sie wollte ihre riesigen Gebiete und Plantagen touristisch erschließen. Dr. Horst Eichler, früher Akademischer Direktor am Geographischen Institut der Universität Heidelberg, war darum einmal mehr in China, um bei der Planung und Vermarktung landesweiter Tourismus-Projekte zu helfen. Gemeinsam mit den Auftraggebern analysierte er touristische Möglichkeiten im Farmgebiet der Teefabrik, erstellte ein Nutzungskonzept und schlug im Gespräch mit Planern, Investoren und Behörden geeignete Marketingmaßnahmen vor.

Ingenieurs für Holztechnik Siegmar Klaus Pfarr in Mexiko ge-worden. Er sollte einer kleinen Schreinerei helfen, mit neuen Produkten weitere Kundenkreise zu erschließen. Der Firmeninhaber und der Senior Experte entwickelten neue Objekte und boten diese mehreren Händlern der Provinz Chihuahua an. Dieses Vorgehen war so erfolgreich, dass die Schreinerei einen Großauftrag über die Ausstattung einer ganzen Einfamilienhaussiedlung mit Küchen und Möbelmodulen erhielt. Um die Anfrage bewältigen zu können, wurde die Produktionshalle nach den Plänen des Senior Experten erweitert und mit gut erhaltenen Maschinen aus Deutschland ausgerüstet. Es war der 13. Einsatz für Siegmar Klaus Pfarr und der 20.000. für den SES insgesamt.

Bolivia: Tips on aircraft safety
Peter Ungemach from Hamburg, a fully qualified ex-Lufthansa engineer, carried out a three-month assignment in Bolivia. His mission was to improve operating procedures, quality and safety in a private aviation company employing 120 people. During the course of his assignment the Senior Expert pointed out that regular maintenance of the aircraft in strict accordance with the manufacturers’ instructions would help to avoid the extra work and time involved in repairs. He also made sure that the burden on the employees, who worked for up to 24 hours at a time, was reduced through a long-term work plan. The Senior Expert worked out many recommendations for the company management regarding safety at work and environmental protection. Communication was mainly in Spanish throughout the assignment.
China: New kinds of eco-tourism
Sleeping in the open air surrounded by fields, hiking in places of great historical significance – eco-tourism has risen sharply in recent years, including in China. But what draws and interests tourists? How can you attract holidaymakers from your own country, and where are they to sleep? The Pangu tea factory in Yudu, South-East China, applied for a Senior Expert to sort out these problems. They wanted to open up their extensive lands and plantations to tourists. So Dr Horst Eichler, former Academic Director of the Department of Geography at Heidelberg University, went to China once again to help with the planning and marketing of national tourism projects. Together with the clients he analysed possibilities for tourism in the farm attached to the tea factory, created a utilisation plan and suggested suitable marketing measures in discussions with planners, investors and authorities.
Mexico: 20,000th SES assignment
The assignment of former wood technology engineer Siegmar Klaus Pfarr in Mexico has turned into a real success story. His mission was to help a small carpentry business to use new products to tap into fresh groups of customers. The company owner and the Senior Expert developed new items and offered them to several traders in the province of Chihuahua. This process was so successful that the carpenters received a large order to equip all the houses in a housing estate with kitchens and other furnishings. In order to cope with this request, the production hall was extended according to plans made by the Senior Expert, and was equipped with good-quality second-hand machines from Germany. This was Siegmar Klaus Pfarr’s thirteenth assignment and the 20,000th for the SES as a whole.

Medical texts are fine as long as they're at the "simpler" end of the spectrum, i.e. if they are aimed at patients rather than medical professionals. For example, I recently translated parts of a website for a private hospital in Switzerland, including pages such as "All about appendicitis" (which required some careful fact-checking) and "What to expect during your appointment".
I also sometimes proofread academic papers written in English by German researchers (these generally come via contacts at German universities): subjects have included sustainable travel, eco-friendly agricultural practices in Africa, and de-icing methods for cycle paths.
I always double-check terminology and usage, and never take on legal or IT-related texts, or anything else that I don't understand.
