What Is Anemia?
Anemia is a condition that develops when your blood lacks enough healthy red blood cells or hemoglobin. Hemoglobin is a main part of red blood cells and binds oxygen. If you have too few or abnormal red blood cells, or your hemoglobin is abnormal or low, the cells in your body will not get enough oxygen. Symptoms of anemia -- like fatigue -- occur because organs aren't getting what they need to function properly.
Anemia is the most common blood condition in the U.S. It affects about 3.5 million Americans. Women, young children, and people with chronic diseases are at increased risk of anemia. Important factors to remember are:
• Certain forms of anemia are hereditary and infants may be affected from the time of birth.
• Women in the childbearing years are particularly susceptible to iron-deficiency anemia because of the blood loss from menstruation and the increased blood supply demands during pregnancy.
• Older adults also may have a greater risk of developing anemia because of poor diet and other medical conditions.
There are many types of anemia. All are very different in their causes and treatments. Iron-deficiency anemia, the most common type, is very treatable with diet changes and iron supplements. Some forms of anemia -- like the mild anemia that develops during pregnancy -- are even considered normal. However, some types of anemia may present lifelong health problems.
What Causes Anemia?
There are more than 400 types of anemia, which are divided into three groups:
• Anemia caused by blood loss
• Anemia caused by decreased or faulty red blood cell production
• Anemia caused by destruction of red blood cells
Anemia Caused by Blood Loss
Red blood cells can be lost through bleeding, which often can occur slowly over a long period of time, and can go undetected. This kind of chronic bleeding commonly results from the following:
• Gastrointestinal conditions such as ulcers, hemorrhoids, gastritis (inflammation of the stomach), and cancer
• Use of nonsteroidal anti-inflammatory drugs (NSAIDs) such as aspirin or ibuprofen, which can cause ulcers and gastritis
• Menstruation and childbirth in women, especially if menstrual bleeding is excessive and if there are multiple pregnancies
Anemia Caused by Decreased or Faulty Red Blood Cell Production
With this type of anemia, the body may produce too few blood cells or the blood cells may not function correctly. In either case, anemia can result. Red blood cells may be faulty or decreased due to abnormal red blood cells or a lack of minerals and vitamins needed for red blood cells to work properly. Conditions associated with these causes of anemia include the following:
• Sickle cell anemia
• Iron-deficiency anemia
• Vitamin deficiency
• Bone marrow and stem cell problems
• Other health conditions
Sickle cell anemia is an inherited disorder that, in the U.S. affects mainly African-Americans and Hispanic Americans. Red blood cells become crescent-shaped because of a genetic defect. They break down rapidly, so oxygen does not get to the body's organs, causing anemia. The crescent-shaped red blood cells can also get stuck in tiny blood vessels, causing pain.

Hypothyroidism, also called underactive thyroid, is when the thyroid gland doesn’t make enough thyroid hormones to meet your body’s needs. The thyroid is a small, butterfly-shaped gland in the front of your neck. Thyroid hormones control the way the body uses energy, so they affect nearly every organ in your body, even the way your heart beats. Without enough thyroid hormones, many of your body’s functions slow down.
How common is hypothyroidism?
About 4.6 percent of the U.S. population ages 12 and older has hypothyroidism, although most cases are mild. That’s almost 5 people out of 100.
Who is more likely to develop hypothyroidism?
Women are much more likely than men to develop hypothyroidism. The disease is also more common among people older than age 60.
You are more likely to have hypothyroidism if you
• have had a thyroid problem before, such as a goiter
• have had surgery to correct a thyroid problem
• have received radiation treatment to the thyroid, neck, or chest
• have a family history of thyroid disease

Source text - English 2.2 Key Concept: Core and Penumbra
Before discussing the causes of ischemic stroke, it is useful to consider the concepts of infarct core and penumbra. These terms were initially given specific scientific definitions. Ischemic penumbra was first defined as underperfused brain tissue at a level within the thresholds of functional impairment and morphological integrity, which has the capacity to recover if perfusion is restored [2]. As applied in the clinic, their definitions have become operational, with the core generally defined as that part of the ischemic region that is irreversibly injured, while the penumbra is the area of brain that is underperfused and is in danger of infracting [3]. These are useful concepts for several reasons. If they can be identified in the acute ischemic stroke patient, they provide prognostic information and may help guide the patient management. Importantly, it is now clear that neuroimaging can provide excellent estimates of the core and the penumbra in individual patients [3, 4] (Fig. 2.3).
To illustrate the core/penumbra concept, let us consider the hypothetical case of an embolus to the main stem portion (M1) of the middle cerebral artery (MCA). The MCA along with the anterior cerebral artery (ACA) arise from the internal carotid artery (ICA) at the base of the frontal lobe (Fig. 2.4). When an embolus lodges in the M1 segment of the MCA, the MCA territory of the brain becomes underperfused (Fig. 2.5). However, in many cases the collateral circulation from the ACA and posterior cerebral artery (PCA) can compensate to some degree. Collateral blood flow to the brain after a large vessel occlusion may occur through the circle of Willis or by communications between small pial vessels on the surface of the brain (pial collaterals). The amount of collateral flow determines the size of the core and the penumbra (Figs. 2.6 and 2.7) [5]. However, it is critical to understand that both the core and penumbra are dynamic entities that depend on the complex physiology that is playing out in the acutely ischemic brain. If the occlusion is not removed, the core size usually increases, while the salvageable penumbra decreases with time. Ischemic penumbra is present for a limited period of time even in the center of ischemia. Irreversible necrosis then radiates to the surrounding tissue over time. The rate of change in the size of the core and the penumbra depends on the blood flow provided by the collaterals [4, 5].
2.3 Risk Factors
In many respects stroke is a preventable disorder. Prevention is the target of a variety of programs to reduce risk factors for stroke. The aim of primary prevention is to reduce the risk of stroke in asymptomatic people. Hypertension, carotid artery stenosis, atrial fibrillation and certain other cardiac conditions, cigarette smoking, diabetes mellitus, dyslipidemia, sickle cell disease, poor diet, physical inactivity, and obesity are well-established risk factors for ischemic stroke [6, 7]. Less well-established risk factors include alcohol and drug abuse, the metabolic syndrome, oral contraceptive use, sleep-disordered breathing, migraine, hyperhomocysteinemia, elevated lipoprotein(a), elevated lipoprotein-associated phospholipase, inflammation, infection, and hypercoagulability (Table 2.1) [7]. The greatest stroke risk, however, occurs in those with previous transient ischemic attack or previous stroke (Table 2.1) [7].

2.3 Factores de riesgo

The most troublesome disease for apple growers in all parts of Minnesota is apple scab. Scab is caused by a fungus that infects both leaves and fruit. Scabby fruit are often unfit for eating, and continued infection of leaves weakens the tree.
Identification
Scab infections on leaves start as olive green to brown spots with an irregular or feathered edge. As leaf infections grow, they may merge together and assume a dark brown velvety appearance. Severely infected leaves may turn yellow and drop prematurely. Scab infections on young fruit start out as olive green to brown spots. As the lesions enlarge, they harden, and eventually become black, corky, inedible areas on the fruit. Severely infected fruit may be deformed and often crack open.
Important biology
Apple scab survives Minnesota’s winters on infected leaves that have fallen to the ground. In spring, spores are ejected from last year’s fallen leaves and create new infections on the young leaves and tiny fruitlets. Leaf spots quickly mature and produce new fungal spores. These spores spread to other leaves to create new infections throughout the growing season.
Abundant rainfall in May and early June is conducive to scab infection, and rainy summers allow the fungus to infect trees even more severely.
Many ornamental crab apple trees are susceptible to apple scab, so the disease can be spread to your fruit trees from nearby flowering crabs.
Management
Depending on where you live in Minnesota, scab control may be very difficult or relatively easy. Growers in drier parts of the state may not have to struggle as hard against scab. An isolated apple planting with no other apple or crab apple trees nearby will also be easier to keep scab-free. Inspecting your trees for scab lesions and keeping records of each year’s infection severity is important in determining how intensively you need to manage scab.
Plant resistant varieties
The best way to deal with apple scab is to avoid it altogether by planting disease resistant varieties. Many varieties of apple trees are resistant or completely immune to apple scab. Susceptible and very susceptible trees will require fungicide sprays every year to control the apple scab fungus. Resistant trees will only require fungicide sprays in very wet years, where the apple scab fungus is nearby in other infected trees or in infected leaf litter. Good sanitation and cultural control practices will minimize the need for even these sprays. Immune trees do not require any fungicide sprays at any time, and will remain disease free all season long.

Finalizada la perforación y terminación (completación), el pozo se encuentra listo para empezar a producir, ya sea por surgencia natural o en forma artificial. El hecho de que lo haga de una u otra forma depende de una variada gama de circunstancias: la profundidad del yacimiento, su presión, la permeabilidad de la roca reservorio, las pérdidas de presión en las proximidades del pozo y en los punzados o en la cañería, etc. Cuantas más dificultades encuentre el fluido en su camino a la superficie, mayores serán las posibilidades de que no surja, o de que lo haga en forma intermitente o a bajos caudales. En esos casos resulta necesario instalar equipos artificiales para posibilitar la producción. En la Argentina menos de un 10% de los pozos producen por surgencia natural.
Los fluidos de un yacimiento –petróleo, gas y agua– entran a los pozos impulsados por la presión a la que están confinados. Si la presión es suficiente, el pozo resultará “surgente”, ya que los fluidos vencerán todos los obstáculos, llegarán a superficie y el pozo producirá sin necesidad de ayuda. Si la presión sólo alcanza para que los fluidos llenen el pozo parcialmente o con un caudal insuficiente, se debe instalar algún sistema que permita elevar los líquidos a la superficie.
La surgencia natural es el método de producción más barato, ya que la energía es aportada por el mismo yacimiento, aunque no siempre es el más adecuado desde el punto de vista de la productividad. La surgencia se regula en la cabeza (boca) de pozo mediante un pequeño orificio cuyo diámetro dependerá del régimen de producción elegido. Los volúmenes de los distintos fluidos producidos varían a medida que transcurre el tiempo. Como regla general, al disminuir la presión del reservorio la producción de petróleo cae, debiéndose recurrir, en esas circunstancias, a equipos artificiales. Con la extracción artificial comienza la etapa más costosa de la explotación del yacimiento. Las cañerías que se utilizan para producir un pozo, tanto por surgencia natural como por medios artificiales, son las mismas. Se las conoce por su nombre en inglés, tubing, y se bajan dentro del pozo en tramos de aproximadamente 9,45 m de longitud, unidos por rosca y cupla, utilizándose distintos diámetros, desde 1,66 a 4,5 pulgadas, según lo requiera el volumen de producción y lo permita la cañería de entubación, casing. En caso de que se quiera producir en forma independiente de dos o más horizontes, deberán instalarse dos o más tubings.
En los casos de surgencia natural, como ya se dijo, el fluido asciende por la tubería sin ayuda externa, utilizando la presión del yacimiento. Cuando la presión del yacimiento no es suficiente se requiere instalar, en estas cañerías, equipos para la extracción artificial. Existen variados métodos de extracción artificial, algunos de los cuales se describen a continuación:
1. Bombeo mecánico. La bomba se baja dentro de la tubería de producción y se asienta en el fondo con un elemento especial.

Either way depends on a variety of factors, such as the depth of the oil field, its pressure, reservoir permeability, pressure losses near the well, and in the perforations or in the tubing, etc. The harder the fluid follows its path to the surface, the more likely it is that it will not flow to the surface, or it will do it intermittently or at low rates. In these cases, it is necessary to install artificial equipment to make production possible. In Argentina, less than 10% of wells produce oil by natural flow.
The fluids in an oil field―oil, gas, and water―are pushed into the wells by the pressure under which they are confined in the reservoir. Flow is controlled at the wellhead or wellbore by means of a small hole whose diameter will depend on the production method chosen. The volumes of the different fluids produced vary as time goes by. Generally, as the reservoir pressure diminishes, oil production decreases. In this case, artificial lift may be used. With artificial lift, the most expensive stage of oil extraction begins. The pipes that are used for well production, both by natural flow and artificial lift, are the same. They are known as tubing, made up of joints which are lowered into the well. The joints are about 9.45 m long, connected by threads and couplings, with different diameters, from 1.66 to 4.5 inches, depending on the requirements of the production volume and casing. In the event of opting for producing oil separately from two or more reservoir zones, two or more tubing strings should be installed.
As regards natural flow, as already mentioned, the fluid is brought up to the surface through the tubing by the pressure in the oil field without any artificial assistance. When such pressure is not enough, it is necessary to install artificial lift equipment in these pipes. There are various methods of artificial lift, some of which are described as follows:
1. Mechanical pumping: The pump is lowered into the production tubing and it is connected to special equipment at the bottom of the well.
