Source text - English Visceral obesity is frequently associated with high plasma triglycerides and low plasma high density lipoprotein-cholesterol (HDL-C), and with high plasma concentrations of apolipoprotein B (apoB)-containing lipoproteins. Atherogenic dyslipidemia in these patients may be caused by a combination of overproduction of very low density lipoprotein (VLDL) apoB-100, decreased catabolism of apoB-containing particles, and increased catabolism of HDL-apoA-I particles. These abnormalities may be consequent on a global metabolic effect of insulin resistance. Weight reduction, increased physical activity, and moderate alcohol intake are first-line therapies to improve lipid abnormalities in visceral obesity. These lifestyle changes can effectively reduce plasma triglycerides and low density lipoprotein-cholesterol (LDL-C), and raise HDL-C. Kinetic studies show that in visceral obesity, weight loss reduces VLDL-apoB secretion and reciprocally upregulates LDL-apoB catabolism, probably owing to reduced visceral fat mass, enhanced insulin sensitivity and decreased hepatic lipogenesis. Adjunctive pharmacologic treatments, such as HMG-CoA reductase inhibitors, fibric acid derivatives, niacin (nicotinic acid), or fish oils, may often be required to further correct the dyslipidemia. Therapeutic improvements in lipid and lipoprotein profiles in visceral obesity can be achieved by several mechanisms of action, including decreased secretion and increased catabolism of apoB, as well as increased secretion and decreased catabolism of apoA-I. Clinical trials have provided evidence supporting the use of HMG-CoA reductase inhibitors and fibric acid derivatives to treat dyslipidemia in patients with visceral obesity, insulin resistance and type 2 diabetes mellitus. Since drug monotherapy may not adequately optimize dyslipoproteinemia, dual pharmacotherapy may be required, such as HMG-CoA reductase inhibitor/fibric acid derivative, HMG-CoA reductase inhibitor/niacin and HMG-CoA reductase inhibitor/fish oils combinations. Newer therapies, such as cholesterol absorption inhibitors, cholesteryl ester transfer protein antagonists and insulin sensitizers, could also be employed alone or in combination with other agents to optimize treatment. The basis for a multiple approach to correcting dyslipoproteinemia in visceral obesity and the metabolic syndrome relies on understanding the mechanisms of action of the individual therapeutic components.

Атерогенная дислипидимия у таких пациентов может быть вызвана одновременно ускоренным синтезом липопротеинов сверхнизкой плотности (ЛПСНП) аpоВ-100, сниженным катаболизмом частиц, содержащих ароВ и усиленным катаболизмом частиц ХВП-ароА-I. Эти нарушения могут являться следствием общего метаболического эффекта инсулиновой резистентности. Важнейшими условиями для устранения нарушений липидного обмена являются снижение веса, физические нагрузки и умеренное потребление алкоголя. Такое изменение стиля жизни может способствовать существенному снижению уровня плазматических триглицеридов и холестерина липопротеинов низкой плотности (ХС-ЛПНП), а также повысить уровень ХС-ЛПВП. Результаты кинетических исследований показывают, что при висцеральном ожирении снижение веса приводит к уменьшению секреции ЛПСНП-ароВ и одновременно к повышению катаболизма ЛПНП-ароВ, что, возможно, связано со снижением массы висцерального жира, повышением чувствительности к инсулину и снижению печеночного липогенеза. Для дальнейшей коррекции дислипидимии часто требуется применение дополнительных фармакологических средств, таких как ингибиторы ГМГ-КоА-редуктазы, производные фиброевой кислоты, ниацин (никотиновая кислота) и рыбий жир. Терапевтическое улучшение показателей содержания липидов и липопротениа при висцеральном ожирении может быть достигнуто при помощи нескольких механизмов, включая снижение секреции и усиление катаболизма ароВ, или увеличение секреции и снижение катаболизма ароА-I. В клинических исследованиях была подтверждена эффективность использования ингибиторов ГМГ-КоА-редуктазы и производных фиброевой кислоты при лечении дислипидемии у пациентов с висцеральным ожирением, инсулиновой резистентностью и сахарным диабетом 2 типа. Поскольку лекарственная монотерапия не всегда способна привести к значительному улучшению дислипопротеинемии, может потребоваться комбинированная фармакотерапия двумя препаратами, например, ингибиторами ГМГ-КоА-редуктазы совместно с производными фиброевой кислоты, ингибиторами ГМГ-КоА-редуктазы совместно с ниацином либо ингибиторами ГМГ-КоА-редуктазы и рыбьим жиром. Для достижения лучших результатов лечения также могут быть использованы новые терапевтические средства, такие как ингибиторы абсорбции холестерола, протеиновые антагонисты передачи холестеринового эфира, сенсибилизаторы инсулина. Основой для комплексного подхода к коррекции дислипопротеинемии при висцеральном ожирении и метаболическом синдроме является понимание механизмов действия отдельных терапевтических составляющих.

Source text - English Объективными симптомами ВСД (вегетативно-сосудистой дистониии) являются: лабильность пульса (тахикардия при незначительной физической нагрузке), тенденция к повышению (гипертонический тип) или понижению (гипотонический тип) артериального давления, лабильность вазомоторов (стойкий красный или, наоборот, белый демографизм, резко положительные или инверсивные ортоклиностатические пробы), холодные кисти, стопы. Кроме того, может наблюдаться так называемый “респираторный синдром”, который заключается в ощущении “дыхательного корсета”, одышке, поверхностном дыхании с ограничением вдоха. Гипертонический тип ВСД характеризуется подъемом артериального давления до 140/90 - 170/95 мм. рт. ст. и нередко выражается пароксизмальной тахикардией, преобладанием симпатоадреналовых реакций, а гипотонический тип вегетативно - сосудистой (или нейроциркуляторной) дистонии - снижением артериального давления до 100/50 - 90/45 мм.рт.ст. с преобладанием вагоинсулярных реакций. Клиническую картину ВСД дополняют: быстрая утомляемость, снижение работоспособности, обморочные состояния и наличие в патогенетическом механизме ВСД - это избыточное образование катехоламинов и молочной кислоты. Накопление последней играет большую роль в кардиальном синдроме вегетативно – сосудистых пароксизмов. Считается более правомерным рассматривать ВСД в рамках не столько самостоятельной формы, сколько синдрома, а факторы, вызывающие это заболевание, группировать следующим образом: конституциональная природа, эндокринные перестройки организма, первичное поражение висцеральных органов, первичные заболевания периферических эндокринных желез, аллергия, органическое поражение большого мозга, неврозы. Синдром ВСД конституциональной природы - проявляется в раннем детском возрасте и характеризуется нестойкостью вегетативных параметров. Отмечается быстрая смена окраски кожи, потливость, колебания частоты сердечных сокращений, артериального давления, боли и дискинезии желудочно-кишечного тракта, склонность к субфебрилитету, тошнота, плохая переносимость физического и умственного напряжения, метеотропность.
Translation - Russian Objective symptoms of vasomotor dyscrasia (VMD) include pulse lability (tachycardia after light physical exercise), a trend to high (hypertonic type) or low (hypotonic type) arterial pressure, vasomotor lability (obstinate pink or, conversely white demographism, strongly positive or inverse orthoclinostatic tests), cold hands and feet. In addition, patients may experience so-called “respiratory syndrome” as a “respiratory corset”, dyspnea, shallow breathing with limited intake of breath. VMD of hypertonic type is characterized by increased arterial pressure up to 140/90 – 170-95 mmHg and is frequently manifested with paroxysmal tachycardia, with prevalent sympathoadrenalinic reactions, while hypothonic VMD is characterized by a decrease in arterial pressure to 100/50 – 90/45 mmHg with prevalent vagoinsular reactions. The clinical picture of VMD includes: rapid fatigue, low performance capability, collapses and by pathogenetic mechanisms characteristic of VMD, i.e. excessive catecholamine and lactic acid synthesis. Excessive accumulation of the latter plays a central role in the cardial syndrome of vasomotor seizures. It is more justified to consider VMD as a syndrome, not an independent disease, while its causative factors can be grouped as follows: organic nature, endocrine alternations, idiopathic involvement of visceral organs, idiopathic disorders of peripheral endocrine glands, allergy, organic lesion of the cerebrum, neuroses. VMD of organic nature develops in infancy and is characterized by unstable vasomotor parameters. Its symptoms include rapid change in skin color, hyperhidrosis, unstable heartbeat and arterial pressure, pain and dyskinesia of gastrointestinal tract, tendency to low-grade fever, nausea, intolerability of physical exercise and mental load, meteotropic reactions.
