Out of pocket expenses Explanation: Firstly, I don't know the context. However, it is terminology that I often encounter in my own work. Basically speaking, the bulk of medical procedures that are conducted here in Japan are covered by the provisions of the national health insurance system. Those that are not have to be paid for by the individual who is being treated. That essentially means "out of pocket expenses." I've attached a reference to the Ministry of Health website below.
