I think both of your options would be fine too (authorisation with an 'h'). Here is another option that may suit the 'minutes-bullet-style' well.

  -> Really? I didn't know that 'proxy' was voting specific. Isn't it just a general term for transfer of authority, as in the dictionary? https://www.merriam-webster.com/dictionary/proxy Also on the website I listed, it is called a 'proxy vote.'

  -> Thanks! OK, yes to multiple parties if one vote to each individually. In practice, the proxy form alternatives are voting, speaking and/ or attendance-only, plus Übertragung shows up as appointment in some past DEU/ENG translation primers.
