If this is from Sénégal, then this might hold a clue. According to a document on their debt collection procedures, types of patients are
"Le particulier à ses frais (P.A.F) : ils payent en espèces ou par virement bancaire" ,
Le malade dispose d'un titre de garantie: c'est soit une lettre de garantie (LG),
- Le client est pris en charge par l'Etat par imputation budgétaire (I.B) : Excepté les agents de santé entièrement pris en charge, ce type de client paie 1/5 de sa facture"
So, the PAF would be a private individual paying at his/her own expense, LG would be a patient with a letter of guarantee from eg a company, and IB (which may be the IMB in your question) which I think is state funded for 4/5 of their bill.

At first, as it's invoicing, a long shot would be "participation au frais", a type of voluntary donation.
Then I also found the more likely " Piste d’Audit Fiable", a billing system.

Was the surgery carried out in Morocco? I ask because someone queries LG on a Moroccan medical bill the other day. In that case, "(LG)" followed "Tél.".
If not, is there a possibility that these three abbreviations indicate different sites for a group of hospitals?

One option, if the "PAF" box is not ticked and leaving it out will do no harm, is to leave it as "PAF (unknown term)", but I expect someone here will know what it stands for.
