Hola Belén, yo lo mantendría en singular porque me suena extraño estimulación inmunitaria sistémica, sin embargo también es muy válido tomando en cuenta que las respuestas inmunitarias pueden ser locales o sistémicas.

Creo que el orden es así: se estimula el sistema inmune (aunque "inmunitario" es más correcto) y esta estimulación produce la inflamación.
Otro problema lo plantea la ambigüedad estructural con la palabra "Systemic"; modifica solo a "immune system" o también a "inflamation"? En cualquier caso la estimulación sistémica del sistema inmunitario en su conjunto va a desencadenar, muy probablemente, una respuesta sistémica.
