診察

Explanation:
少し意訳かもしれませんが、一連の流れからしてここでのencounterは診察のことではないでしょうか。その後に続くObservation, Enquiry, Medicineを見ると、診察行為の一連の流れに相当するような気がします。診察をして、経過（様子）を見て、最後に薬を処方する、ということではないでしょうか。
