disagree  B D Finch: You seem to have been confused by the omission of the hyphen in "evidence based practice", which should read " evidence-based practice". Even without the hyphen the syntax makes it clear that this is practice based on evidence, not the other way around!

Note that, if you take the whole relevant section: "three areas described //by ASHA (www.asha.org/Research/EBP/)// as making up evidence based practice," it becomes clear, by removing the intervening phrase "by ASHA (www.asha.org/Research/EBP/)", that the main clause is: "three areas described as making up evidence based practice". The preposition "as" belongs to "described".
