Source text - Indonesian Pemeliharaan kesehatan Karyawan dan keluarganya adalah tanggung jawab Karyawan masing-masing. Namun demikian, untuk membantu Karyawan dalam memelihara kesehatannya, Perusahaan memberikan fasilitas kesehatan berupa penggantian biaya pengobatan dengan ketentuan sebagaimana diuraikan dibawah ini.
Fasilitas kesehatan yang diberikan oleh Perusahaan kepada Karyawan dan keluarganya akan menggunakan sistem penggantian (reimbursement), dimana Karyawan akan membayar terlebih dahulu biaya kesehatannya dan Perusahaan akan melakukan panggantian berdasarkan ketentuan yang berlaku.
Penggantian batas maksimum biaya perawatan per tahun, secara otomatis mengikuti perubahan status karyawan.
RAWAT JALAN
Ketentuan mengenai Fasilitas Rawat Jalan diuraikan sebagai berikut:
a. Besaran batas penggantian maksimum biaya pengobatan per tahun (dari Januari sampai dengan Desember) bagi Karyawan adalah 1 (satu) bulan upah.
b. Tambahan biaya Rawat Jalan atas kelahiran anak karyawan, berlaku efektif sejak karyawan menyerahkan Surat Keterangan Lahir dari Rumah Sakit / Klinik Bersalin ke HRD.
c. Penggantian biaya pengobatan untuk Karyawan, Suami/Istri dan Anak dapat saling dipergunakan untuk Karyawan, Suami/Istri dan Anak.
d. Besaran maksimum penggantian untuk karyawan yang bekerja kurang dari 1 (satu) tahun, akan dihitung secara proporsional sebesar 1 (satu) bulan upah.
e. Bagi karyawan yang masih dalam masa percobaan (probation), penggantian biaya rawat jalan selama masa percobaan bisa di klaim setelah dinyatakan lulus dari masa percobaan dan diangkat menjadi karyawan tetap.
Translation - English The health maintenance of the employees and their families is the responsibility of each employee. However, to assist employee in maintaining his health, the company provides health facilities in the form of reimbursement of medical costs with the provisions as set forth below.
The health facilities provided by the company to the employee and his family will use the replacement system (reimbursements), where the employee will pay in advance the cost of his health and the company will do reimbursements based on conditions.
The maximum limit of the medical costs to be reimbursed per year will change automatically following the changes of the employee’s employment status.
OUTPATIENT
Provisions on Outpatient Facilities are outlined as follows:
a. The maximum limit of the medical costs reimbursement per year (from January to December) for employees is 1 (one) month wage.
b. Additional Outpatient charges over child birth of the employees is effective since the employees handed over the Certificate of birth from the Hospital/Maternity Clinic to HR Department.
c. Reimbursement of medical costs for employees, spouses and children can be mutually used for employees, spouses and children.
d. The maximum limit of the medical cost reimbursement for employees working less than 1 (one) year, it will be calculated proportionally in the amount of 1 (one) month wage.
e. For an employee who is still on probation, reimbursement of the cost of outpatient during the probation period can be claimed after passing the probation and being appointed as permanent employee.
